The CMS should transition away from a fee-for-service model and factor costs into coverage decisions to reduce spending without harming quality, a Rand Corp. study found. Congress has barred the CMS from considering costs of providing care or services in determining coverage, but the Affordable Care Act's accountable care provisions aim to shift the nation's health care system away from fee-for-service.
