The Wyoming Institute of Population Health, a unit of Cheyenne Regional Medical Center, is establishing "medical neighborhoods" made up of tightly coordinated networks of medical practices, hospitals and pharmacies. The program uses telemedicine technology and regular patient data reports, and it includes patient-centered medical homes. The initiative is projected to save patients $33 million in health care costs over three years. The PCMHs emphasize prevention and wellness, coordinate care, and provide care transition plans.
