Insurers and health care providers have embraced the promise of value-based care, but the success of the transition depends on the ability to address social determinants of health, according to Brian Donovan, vice president of network management at Health Partners Plans. A reimbursement structure that accounts for social determinants, data tools that identify needs and community partnerships are critical components, Donovan said, and SDOH programs must also account for patients who do not regularly see health care providers.
