Just half of employees in a large academic radiology department said they always reported safety concerns, according to a study in a radiology journal. Researchers found that a reporting threshold that was too high was the most prevalent barrier to speaking up about safety issues, and radiology residents and fellows had the highest rate of not always reporting safety concerns.
