Hospital's transition program reduces readmissions, cuts costs
A transitional care program launched by the Veterans Hospital in Madison, Wis., has reduced hospital readmissions by a third and saved hundreds of thousands of dollars, according to a study published in Health Affairs. Nurses make weekly follow-up calls to discharged patients for four weeks as part of the program to help provide continued care until patients are able to visit with a primary care doctor.
